We are constantly striving to identify hidden carers through a number of initiatives including HUB meetings and the Young Carers Initiative.
HUB meetings bring together workers from the Health and Social Care Partnership and third sector organisations like ourselves. By coming together, we are able to identify who is best positioned to give assistance to people as they are being assessed by Social Services. This has allowed us to help carers earlier, at a point when they may be feeling overwhelmed and have questions we can help them with.
We are delighted to be funded 2 Hospital Discharge Workers who work closely with the Discharge Without Delay team within hospitals through-out East Ayrshire. John who is based within Ayr Hospital and Lorna who is based within Crosshouse Hospital. Carers are often overlooked when their loved one is admitted to or discharged from hospital. John and Lorna are able to ease this process and make sure that Carers and their wider families are included, supported and more informed during the admission and discharge process. We happily give presentations to other groups and organisations and regularly deliver presentations to school pupils about what it means to be a carer.
